2022 NEW Vision Carrier/Plan - Guardian Vision
This information does not apply to Chemung County CSEA, Sewer District employees or Retirees enrolled in Medicare PPO Plan. 

Lifetime Benefit Solutions (LBS) is no longer providing vision plans and our last day of coverage with this plan will be December 31, 2021.
  There will be a runout period where claims incurred through December 31, 2021 will be processed through February 28, 2022.  Please be advised, any claim received after February 28, 2022 will not be processed and will not be paid.  It is very important your providers bill any outstanding claims prior to this date.

Guardian has been selected as the carrier of the new vision plan beginning January 1, 2022.  Click here to see a preview of the summary of benefits.  Please check back here for further details.  ID cards will be mailed out from Guardian to your home.

Effective January 1, 2022, the new Guardian vision plan allows eligible dependents to stay on this vision plan through age 26 without regard to student status.  If you have dependents between 19-26 (who are currently on your medical plan) that you wish to re-enroll in the vision plan, please complete the Guardian enrollment form here and return to the Insurance Department.  Your dependent will be re-enrolled January 1, 2022 OR the first of the next month the form is received.


Retirees Enrolled in Medicare PPO Plan - NEW Carrier/Plan 2022 United HealthCare (UHC)

United Health Care (UHC) Medicare PPO Mail Order and Prescriptions
Please be advised members enrolled in the UHC Medicare plan will no longer be able to utilize Express Scripts for their mail order. UHC's mail order is OptumRx, which you will see referenced on the bottom of your ID card.

Please call the customer service number on the back of your ID card to learn how to get set up with OptumRx.  You can choose to fill your Rx at a participating pharmacy and pay 2 copays for a 90 day supply OR choose to utilize Optum Rx mail order and pay 1 copay for a 90 day supply.

You may call UHC customer service number (800) 457-8506 8am-8pm local time, to go over your prescriptions so you know what tier copay they are in or any special information you may need to know.  You can also find out about benefits, participating pharmacies, providers, fitness plan or any other coverage questions.


Excellus Dental - with all the various changes occurring for 2022, please be advised that your Dental is still through Excellus.  You will continue to use the same ID card.


Excellus will be issuing new ID cards with our renewal 1/1/22 to our membership.  Some members may receive the new ID cards early if they have transactions before January.  The new ID cards will be issued to each member of your family enrolled on the policy.  The subscriber and ID# will be on each ID card; however, the dependent's name will appear on their own ID card.

DENTAL TPA CHANGE EFFECTIVE 7/1/2020
Lifetime Benefit Solutions (LBS) has been the Third Party Administrator (TPA) for Chemung County's dental plan; however, LBS is no longer providing this service in their line of business.  Excellus has taken over as the TPA for Chemung County's self-insured dental plan effective 7/1/2020.
