Health Department requests residents' assistance by participating in an online health survey

ELMIRA- The Chemung County Health Department is asking for assistance to improve the health of residents’ in our area by completing an online survey. The 15-minute survey will allow residents to share their experience with health services in their community. The survey can be found at

“Your personal health story can help improve lives. It is important for us to hear from as many residents as possible in order to plan our future public health initiatives and to direct the focus of our activities,” stated Peter Buzzetti III, Chemung County’s Public Health Director.

“These stories will help identify health-related needs and reveal where programs and policies can make our communities healthier,” explained Trilby de Jung, chief executive officer of Common Ground Health, the region’s health research and planning organization.

Participants can complete the survey anonymously, or enter their name and email for a chance to win a $500 gift card to a local supermarket or gas station. See official rules here. Participation is limited to residents of the following Finger Lakes, N.Y. counties: Chemung, Livingston, Monroe, Ontario, Schuyler, Seneca, Steuben, Wayne and Yates.
